Subscriber Checklist
for Interment or Scattering of Cremains
in the Memorial Garden

  1. A prospective Subscriber should start by becoming familiar with the UUC Memorial Committee Home page and then contacting the Congregational Liaison .

  2. The Subscriber and Liaison will complete the appropriate Proof of Purchase form. The original and a copy of the form will be given to the Subscriber, and a copy will be retained in the records of the Memorial Committee. The Subscriber's copy will be given to the Alternate Subscriber as designated on the form.

  3. If the Subscriber purchased the rights for him- or herself, upon the Subscriber’s death, the Alternate Subscriber takes on the role of Subscriber.

  4. When the Subscriber is ready to inter or scatter the cremains, he/she presents a copy of the Death Certificate to the Congregational Liaison.

  5. For a niche interment, the Subscriber will select the wording for the niche nameplate according to the guidelines on the Columbarium Niche Nameplate Caption Form. The Liaison is responsible for placing the order with the engraver.

  6. Biographical information for the decedent to be preserved in the Memorial Registry may be prepared in accord with the Committee Policies and Procedures (Section 8).

  7. Any service or reception are up to the Subscriber to arrange. The UUC Meeting House, as well as the Garden, may be scheduled for a service and/or reception. Check with the UUC Administrator for available dates and times.

  8. The Subscriber should coordinate with the Columbarium Caretaker as to the date and time of the interment or scattering. If a niche is to be used, the Subscriber should consult with the Caretaker about the proper receptacle for the cremains. Usually, the cremains are transferred to one of the niche canisters prior to insertion into the niche, but a Subscriber-provided urn (maximum diameter 5-7/8” and maximum length 8”) may be used. The Caretaker can help with the transfer of cremains from the crematorium container to an appropriate canister or urn.

  9. For a niche interment, the Caretaker (a member of the Memorial Committee) will open the Columbarium niche to receive the cremains prior to the ceremony and close it following the ceremony and set the nameplate in place. For scattering, the Caretaker will set the paver before or after the ceremony — if it has arrived from the engravers. The Caretaker will provide instructions beforehand if the Subscriber desires to set the paver during the ceremony.

  10. After the cremains are interred or scattered, the Liaison and the Subscriber will sign the appropriate Memorial Garden Certificate on which the memorialized person’s name and the interment/scattering date has been inscribed. The Certificate will then be presented to the Subscriber.
